Myerlee Pharmacy makes it convenient to refill your prescription online for pick up in our pharmacy. Please use the form below to complete your prescription refill request. For questions or concerns, contact one of our pharmacy staff members at 239-482-3022. Prescription refills require the initial prescription from your doctor to be on file at Myerlee Pharmacy.

Myerlee Pharmacy provides services only to patients with a valid Florida residential address. While medications may be shipped out of state, prescriptions must originate from a Florida-licensed provider and comply with all applicable destination state laws.
